Nurse Practitioner/PT (20+ hours per week) In collaboration with the Chief Medical Officer and other staff, she/he is responsible for the assessment, planning, implementation and evaluation of palliative care interventions and providing follow-up for patients during phases of illness. Licensed to practice in state. Certified as a Clinical Nurse Specialist or Nurse Practitioner. Certification by a national body (ANCC, NBCHPN, AANP). Minimum of five years nursing experience; clinical nursing care in geriatrics, long term care and/or hospice. Palliative care ARNP preferred. Knowledge of the interdisciplinary team concept.
Social Worker (Clinical): MSW required; experience in gerontology and medical social work preferred; exceptional communication and organizational skills to work in collaboration with members of the interdisciplinary team. Will provide psychosocial assessments, direct casework and counseling services to patients and families. Current state license required. This is a full time position with benefits.
RN Case Manager Successful candidate must be RN licensed in Virginia, 2+ years relevant experience required along with demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation and case management. Hospice experience preferred but not required.
On Call Nurse (PRN weekend days) Assures the delivery of high quality patient care through timely and responsible management of patient/family needs by consistently responding to HOPs answering service via beeper and/or phone to triage calls. Maintains log for calls received. Makes home visits to assess patient needs, provide nursing care and emotional support to families. Must be RN licensed in state of VA with 2+ years experience.
Triage Nurse (PT evenings 5:00 - 10:00 p.m., Monday-Friday or PRN weekends 8:00 a.m. - 4:00 p.m., Sat. & Sun.) Assists on-call staff in responding to patient/family needs in accordance with plan of care. Must be RN licensed in state of VA with 2+ years experience.
Certified Nursing Assistants (CNA) to serve patients in Charlottesville and surrounding counties. As a member of the health care team, candidate will provide personal care and attendant services ensuring comfortable, safe, total patient care. Must have current Virginia certification.
PRN positions are also available in the field as well as at our Hospice House for the following disciplines: RN, LPN, or CNA.
